Sending forms to HR

In order to protect personal and other sensitive information, all Human Resources forms must be submitted using the secure drop box. No form submissions will be accepted by email. 

Visit the forms site and use the list and search function to find a specific HR form.

Please be aware that the Human Resources forms site only has forms related to HR actions. The following sites contain additional forms that employees may need:

• Controller and Payroll
• Faculty (for T&R, Research, and A/P faculty)
• Parking
