Premium Rewards
Employees who are covered by Anthem COVA Care and Aetna COVA HealthAware plans are eligible for the Premium Rewards Program. Participants receive a discount of $17 per month. Spouses on the plan are also eligible for a $17 per month discount.
To receive Premium Rewards, visit the COVA Care or COVA HealthAware plan website to access your online health assessment. Step-by-step instructions for completing the assessment can be found here.
The reward will be effective the first of the month, approximately six to eight weeks, after the online health assessment is completed and will continue through the end of the plan year, which is June 30 each year. The following chart provides a schedule of effective dates based when the Health Assessment is completed:

Keep in mind that this shows when the reward becomes effective. You should see it in your deductions on the paycheck that follows the date listed.
Note: The health assessment needs to be updated annually - usually during open enrollment - to continue the rewards for the next plan year.
