The Panel of Physicians form was most recently updated on March 1, 2024. Please note that this is not a comprehensive list of all specialties. For listings of providers within a certain specialty, or within a different geographic region, please visit the Careworks website.

Please note that medical care administered at Schiffert Health Center is not approved for Worker's Compensation.

General Procedures

• Employees are required to immediately inform their supervisor or other department personnel of a work-related injury or illness. Following the report of an injury, departments must immediately complete the Employer's Accident Report. Employees must select a physician from the panel of physicians for treatment by completing the Panel of Physicians Form. The Panel Selection Form must be returned to the Division of Human Resources.

Virginia Law Requires the following:

• An Employer's Accident Report filed within five days of notification of job-related illnesses or injuries.
• A choice of physicians from the Workers' Compensation Panel of Physicians Form.
