Select one of the following link to view the current Panel of Physicians forms and last revised date.
These forms are not a comprehensive list of all specialties. For listings of providers within a certain specialty or within a different geographic region, please visit the Careworks website. Employees outside the Blacksburg, Roanoke, or Greater Washington, D.C. area should use the Careworks site to find panel physicians in their area.
Note: Medical care administered at Schiffert Health Center is not approved for Workers' Compensation.
General Procedures
- Employees are required to immediately inform their supervisor or other department personnel of a work-related injury or illness.
- Following an injury report, departments must immediately complete the Employer's Accident Report.
- Employees must select a physician for treatement from the Panel of Physicians and complete the Panel of Physicians form. This form must must be returned to the Division of Human Resources; submit the form using the secure HR dropbox.
- Other departments not located in the Blacksburg, Roanoke or Greater Washington D.C., employees should go to Careworks and search for a listing of physicians in their area. Once they select a physician, the employee should complete and sign the Panel of Physicians Form; employees may use either form above.
- The physician or hospital should submit bills to:

Virginia Law Requires the following:
- An Employer's Accident Report filed within five (5) days of notification of job-related illnesses or injuries.
- An employee's choice of physicians from the Workers' Compensation Panel of Physicians form.
