Discipline/Performance

Follow the process below for any faculty or staff member that has a change in duties or a decrease in salary due to performance issues. Departments may contact Human Resources if they have questions about how to identify appropriate changes in position.

Faculty Procedure

The following procedure applies to Administrative and Professional, Teaching and Research, and Research Faculty positions.

  • Complete P3A form
  • Signature Requirements
    • Department Head or Director
    • Senior Management
    • Compensation
    • Human Resources
    • If Research - OVPRI
  • Additional Documents
    • Create/Update Position Description. The Division of Human Resources will assign the position number to the requested action. Approval of the request will be document in the applicant tracking system
    • Terms of Faculty Offer will be provided by Human Resources

Staff Procedure

  • Complete P3A form
  • Signature Requirements
    • Department Head or Director
    • Senior Management
    • Compensation
    • Human Resources
    • If Research - OVPRI
  • Additional Documents
